I will be unexpectedly out of the office until September 14, 2026.

I will not be checking email, voicemail nor fax during my absence.

Prescriptions

Please advise your pharmacy that they cannot initiate a request through my email, e-prescribing, voicemail nor fax during my absence.
 
You will need to initiate the refill request by contacting Dr. Bogan, who will submit the prescription refill to your pharmacy.
 
Please contact Dr. Bogan at least a few days before you run out of medications.
Please include:
--Full name
--DOB
--Name of medication, strength and how you are taking it
--Pharmacy phone number

I will resume appointments on September 14.
If you have a scheduled appointment before then, it is now cancelled.
