In pursuit of these goals, and in accordance with guidelines set forth by the Colorado Division of Insurance, DHMP is collecting demographic data from its members and providers about their race, ethnicity, sexual orientation, gender identity, sex assigned at birth and ability status.
Please note:
The demographic data collected will be confidential, deidentified and treated as Protected Heath Information (PHI). Any voluntarily reported demographic data will be used in aggregate only. The data will be used to improve racial health equity, reduce health disparities for covered persons who experience higher rates of health disparities and inequities and provide aggregate information regarding the demographic diversity of DHMP's covered population and providers.
