Sim Center Trainer/Equipment Checkout Request Form

This form is for Hennepin Healthcare employees and affiliates to request the use of Hennepin Healthcare Interdisciplinary Simulation and Education Center (ISEC) procedural task trainers or equipment.

Please fill out the following request form to check out a trainer or piece of equipment to be used outside of Hennepin Healthcare ISEC (Interdisciplinary Simulation Education Center)

We request filling out this form at least 2 weeks prior to your requested date(s) as our schedule gets filled quickly. This will provide us with sufficient time to plan and prepare the equipment for pick up. Additionally, if we are unable to accommodate this request, it allows enough time for us to suggest alternative options if available.

Expectations when borrowing equipment from the Sim Center:

• Wear gloves at all times when using trainers
• If a trainer is not working properly or breaks, please notify us to troubleshoot. DO NOT attempt to fix yourself.
• Refilling fluids in trainers is for ISEC staff only. 
• Please clean the trainer of any excess lubricant or gel with a damp or dry washcloth, followed by a sani wipe. 
• When aspirating fluid from a trainer, you must inject any withdrawn fluid back into the trainer without introducing air. 
• No food or drink is allowed around trainers or equipment. 
• Keep our trainers tattoo-free by keeping all ink pens at least 5 feet away from the trainers.
• 18 gauge or smaller needles are allowed in trainers. No larger needles are to be used. 
• NO scalpels are to be used on mannequins unless otherwise directed.
• All Simulation equipment and supplies are intended for simulation use ONLY and must be kept out of clinical environments.  Many of our supplies are defective or outdated and thus are NOT safe for patient use.
• NO alterations to the trainer unless authorized by ISEC staff.
• No use of any iodine on any trainer.  Chloraprep or alcohol are acceptable.
• The trainer/equipment must be secured and under your supervision at all times while out of the sim center.
