Outpatient Mental Health Programs Referral

Common reasons to refer to one of our outpatient programs

  • The patient would likely regress if not in this highly structured setting and may otherwise require hospitalization.
  • The patient is not progressing in outpatient therapy and a higher level of care is indicated.
  • The patient is experiencing an acute crisis.
  • The patient is in need of medication evaluation, treatment, and monitoring.
  • The patient is in need of further diagnostic clarification and evaluation.
  • The patient needs more structure and support than they are currently receiving.


Thank you for your interest in the intensive outpatient mental health programs at Hennepin Health. Please complete the referral as thoroughly as possible.

If there is additional information you would like us to know about the person you are referring (DA, progress notes, etc.), you may fax that to the number below or call the program.

When we receive your referral, we will contact the patient to schedule an intake. All patients are required to complete a diagnostic assessment during this intake to determine if the program is appropriate for the patient at that time. These programs are completely voluntary and patients must express a desire to participate. The referral alone does not constitute admission to either program. At intake, patients will be assessed for the appropriate level of care and will be considered for both programs.

Please note that Medicare does not cover Day Treatment services. Patients will need additional coverage in addition to Medicare in order to attend DTP.
