Warranty / Dental Maintenance

Dear Patient,

This letter is intended to inform you of our office policy regarding warranties that have been in place for several years.

We do provide a 5 year warranty from the date of delivery on all crowns, veneers and onlays.

We also provide 1 year warranty from initial date on fillings.

All these warranties are in effect provided the patient has kept up with scheduling the recommended cleanings, exams and x-rays. This warranty becomes void if a patient does not keep up with their recommended re-care appointments, exams, and x-rays for any reasons. Also, if a Bite Guard is recommended due to grinding or other parafunctional habits and one declines to get this necessary appliance, the consequent outcome of breakage or fractures of the restoration will not be covered by this warranty.

If you are recommended more frequent hygiene appointments than covered by your dental benefit plan, to maintain periodontal health, it is necessary to follow this recommendation for warranty purposes.

This letter’s intention is to inform you of the importance of keeping your dental hygiene, exams and x-rays current and so that we can monitor the dental treatment and services we have provided for you, to maintain a healthy smile.

These warranties are not the norm in health industry, and they reflect the pride we take in or products and services.

This warranty only covers work redone at our office and if you are a patient of record at the time the services are needed. If you have been dismissed or left our practice, this warranty is void.
